When it comes to hypospadias repair, there are different philosophies among surgeons when it comes to the aesthetics of the glans, penile skin and scrotum. For example, this patient's surgeon did not attempt to place the meatus within a position normally enclosed by the glans, choosing to leave it below the head of the penis is a distal hypospadias position. This almost certainly contributes to the urinary spraying.
