| Video Discription | Dr. Bush and Dr. Snodgrass of the Hypospadias Specialty Center show their approach to teens and adults with uncorrected or persistent ventral penile curvature. This is a very common problem in adults with failed proximal hypospadias repairs. It is also seen in men with uncorrected or failed prior correction of congenital penile curvature (CPC). For men with curvature more than 30 degrees after degloving, they use the STraighten And Close (STAC) technique. In this unedited video, they demonstrate how to perform 3 ventral corporotomies in a post-pubertal patient, as well as harvesting a dartos flap to cover these corporotomies. The urethrostomy is created, and then attention is turned to correction of penoscrotal transposition and skin repair. A huge benefit to STAC repair is that the now-straightened penis acts as its own tissue expander, stretching existing penile skin which is typically very short on the underside. The penoscrotal junction should be tacked internally to help with penis skin stretching. The addition of hyperbaric oxygen therapy and postoperative betamethasone also help rehabilitate the penile skin flaps. |
