In this video, we present step-by-step instructions, followed by outcomes, for a female patient with urethral stricture, which we treated utilizing a vaginal free graft as a dorsal onlay. METHODS A female patient with symptomatic urethral stricture was selected for treatment utilizing a vaginal free graft. Consent for recording video during surgery was obtained. Preoperative and postoperative voiding parameters, and quality of life indices, were recorded. RESULTS At 3 weeks following vaginal free graft dorsal onlay urethroplasty, voiding cystometrics demonstrated a maximum flow rate of 39 ml/s, with a post void residual of 0 ml. At 2 months following surgery, the patient was asymptomatic. AUA symptoms score was 3, and quality of life was 1. Voiding cystometrics revealed a maximum flow rate of 14 ml/s.
