Rhinoplasty specialist Dr. Robert Caridi at Westlake Plastic Surgery shares a video with a common request - "Make my nose smaller and more feminine!" In this case, the patient wants a smaller, more feminine nose that fits her face while balancing and preserving her ethnic features. While her other facial features are amazing, her nose is large in every dimension, from a large tip and cartilages to an overgrown septum that pushes her tip down even further.
It is important to be aware that there is only so much that can be achieved with a nose job, as each patient’s specific nasal anatomy provides its own limitations that can lead to undesirable results if ignored. For this patient, the amount of skin and underlying nasal anatomy she has is what dictates the extent of changes which can be made. However, by reducing the hump, refining the tip, including correcting her “hanging columella”, and reducing the width and height of her nose overall, the patient will have a drastically more desirable nose that will simply fit her face better and complement her other facial features.

This nose job surgery footage video is an educational and detailed presentation of a rhinoplasty being performed in the operating room, including the pre-operation anatomic analysis between the patient and Dr. Caridi.
Beginning with a stair step incision on the columella, followed by a rim incision, exposure of the cartilages is achieved. Next, the projection, or hump, of the nasal ridge is reduced using a generous approach due to the overall amount of reduction needed of the patient’s nose. An osteotome is used to further reduce the bone. The bone which is still dominating the nose is then reduced with a rasp, which is effectively a filing of bone and reducing the height of the nose. Dr. Caridi utilizes a conservative approach and incrementally reduces the nasal ridge to a more refined, slightly curved appearance that is preferable among females. He then performs a percutaneous osteotomy, which is the breaking of the nose bone to narrow the appearance of the base of the nose. Finally, the nose is closed with sutures whose tension can be adjusted, that along with dorsal splints and a tip dressing for support, will get the cartilage set in a position that will help achieve the best results during the rhinoplasty recovery.

After surgery, Dr. Caridi reviews the results of the rhinoplasty procedure and notes that the patient’s nose is smaller and shorter overall, the tip is much more refined than before, the nose is narrower than before, there is balance and ideal proportions, and the patient’s nose is simply a much better sculpted, smaller and more feminine nose than before.
At her 1-week post-op appointment, the patient’s nose splint is removed and she can’t help but exclaim “Oh my god! It’s so cute!” As time progresses and her swelling, which is substantial, subsides, her rhinoplasty results will only get better and better.
