| Video Discription | Dr. Nicol Bush and Dr. Warren Snodgrass at the Hypospadias Specialty Center demonstrate unedited video of a penoscrotal hypospadias repair. The first step in repair is what we call STAC 1 - STraighten and Close. In this video, you will see the initial skin incisions, the appropriate planes of dissection for the initial degloving, and how to assess penile curvature after degloving. To straighten the penis, 3 corporotomies are made, which are then covered with a dartos flap. A urethrostomy is created, the cleft scrotum is repaired, and foreskin reconstruction is performed. At STAC 2 repair, performed 6 months later, this patient will have a urethroplasty graft placed in preparation for creation of a neourethra once that has healed. This patient has very typical features of penoscrotal hypospadias, with ventral penile curvature ('chordee') that measures 75 degrees. He also has short ventral penile skin as well as penoscrotal transposition and scrotal clefting. The first surgery, which addresses each of these issues, is critical to setting up a healthy, straight penis with symmetric skin and a normally-positioned scrotum in order to minimize complications when the new urethra is created. |
