| Video Discription | Sometimes, the reconstructed ventral skin flap can tether after STAC 1 repair. When it does, the penis can have residual curvature. In this short clip, Dr. Bush and Dr. Snodgrass of the Hypospadias Specialty Center demonstrate how they approach a STAC 2 repair when this is the case. They show mobilization of the skin, mobilization of the native urethra, and a plication for the residual curvature present at this stage. Recent review of our data in over 200 recent patients who have undergone 3 corporotomies demonstrate that 85% are straight at STAC 2, and the other 15% have less than 30 degrees residual curvature that is straightened in this manner. These patients had an average of nearly 70 degrees of curvature, with 1 out of every 3 having 90 degrees or more. This is a short video that just takes you through the straightening steps. The remainder of harvesting the graft and securing it in place can be seen in our prior STAC 2 prepucial graft videos. If during a STAC 2 repair you encounter more than 30 degrees of curvature after release of the skin and urethra, then evaluation of technique during your corporotomies is suggested. It is likely better to redo the STAC 1 instead of moving forward with grafting in this circumstance. |
