| Video Discription | A male patient with IDDM and HTN presented with weight loss. CT showed proximal pancreatic ductal dilatation c/w IPMN and a 4.5 cm distal pancreatic mass obstructing splenic vein causing gastric fundal varices. This was a messy total pancreatectomy case that required significant preoperative planning effort (in regard to operative sequences, robotic setup, patient positioning, etc.) because of patient’s high BMI, excessive visceral fat, narrow rib cage but wide belly. I decided to divide the procedure into 2 parts: 1- proximal and central pancreatic dissection & resection + reconstruction (hepaticojejunostomy and duodenojejunostomy), 2 - distal pancreatosplenectomy (oblique right lateral position). |
