| Video Discription | Hi friends ,this is Dr. Vikas Sinha Professor ENT and today I am showing the video of KITE V-Y FLAP for the Basal cell carcinoma This middle aged patient had histopathology proved basal cell carcinoma and V-Y flap surgery was done. The V-Y flap is an advancement flap.The marking of lesion of basal cell carcinoma was done with marker pen with clear 0.5 cm clear skin safety margin and V shaped flap was drawn with the marker pen. The xylocaine with adrenaline was infiltrated at the lesion and at the flap design. A circular incision was given around the lesion of basal cell carcinoma with 0.5 cm clear skin safety margin and lesion was removed which was sent for the histopathology examination again. All the bleeding points were cauterized with the cautery.The incision was extended V shaped at the flap as per flap design. The incision was deepened and all the edges of this island flaps were made free so it can be pushed up . The upper edges of circular lesion was also made free . The island flap was pushed up with the digital pressure and sutured with the upper edges of circular incision and it is sutures in two layers. Two horizontal incision is given on the either side of the circular lesion so that corner edges of island flap apposed appropriately with the adjoing skin thus preventing any cutaneous irregularity at this point. The extra redundant skin is excised.The skin edges of island flap is sutured with adjoing skin with non absorbable suture material. The suturing is started from the upper part of flap towards its apex. After complete closure the V shaped incision becomes Y shaped. The dressing is applied over it. |
