| Video Discription | Less than 10% of renal cell cancer (RCC) have an inferior vena cava (IVC) involvement, and in only 10% of them the progression gets the right atrium (level IV). A clinical case of a 61-year-old woman with a RCC level IV is showed. The surgical approach was a median sternotomy with a midline laparotomy extension. The retrohepatic left-side IVC was exposed through the lesser omentum as an adequate and alternative way for the retrohepatic IVC exposure. An in-block right kidney and IVC-right atrium thrombus was resected. Cardiopulmonary bypass without cardiac arrest was used for this resection. The IVC wall defect was repaired with a bovine pericardium patch. The immediate postoperative course was succesful. After 18 months the patient was alive, although she developed systemic metastases. |
