Professor of Surgery Athens Medical School The video depicts a case of transanal total mesorectal excision (ta-TME) for a mid-rectal cancer from our team. The ta-TME preceded the laparoscopic step of the operation. That facilitated the caudal-cranial and medial-lateral dissection at the laparoscopic phase. After positioning the Lone star retractor, a purse-string suture was placed through the rectal mucosa to tightly occlude it distally to the lesion. Positioning of the Gelpoint path Transanal Access Platform (Applied Medical Inc) took place. CO2 was insufflated to a pressure of 12mmHg. A peri-mesorectal dissection was performed. Assistant’s finger in the vagina aided in guiding dissection at the rectovaginal septum plane. The peritoneal cavity was entered and the laparoscopic dissection was followed. Extraction of the specimen via a suprapubic incision was followed by inserion of the anvil of the HEM3348 (Covidien EEA Hemorrhoid and Prolapse Stapler 33mm-4.8mm). The distal purse-string was placed at the anal muscosa and was stabilized by passing the needle through one of the openings of the anvil’s long shaft. The colo-anal anastomosis was performed with the circular stapler HEM3348. |
