| Video Discription | A "how to do a tracheostomy": As a rhinology/skull base surgeon, we often look after patients with complex tumours of the paranasal sinus and skull base. We work closely with neurosuregons and provide tracheostomy service and care. This video is primarily intended for surgical trainees to see how a real surgical tracehostomy is performed. This patient is intubated and being coverted to a tracheostomy but the technique is identical to what we perform under local anesthetic. The key to procedure here is: 1. local anesthetic infiltration down to and into the trachea 2. positioning 3. midline separation of the muscles in the fat plane and avoiding wide dissection as this creates unnecessary dead space around the final tube 4. division of the thyroid isthmus - rather than trying to awkwardly work around it 5. simple slit incision only between 2nd and 3rd trachea rings 6. suturing the base plate with 2/0 silk to secure for the first 7 days until a well formed track is established and tube changes are possible/easy.
