| Video Discription | Complex fistula in ano is one of the most challenging scenarios to manage by surgeons. usually, two main concerns while managing complex fistula in ano, recurrence, and incontinence. there are many sphincter preserving procedures but we prefer LIFT as it is easy to learn and also to reproduce. but the addition of A LASER to the LIFT procedure will help to take care of the distal tract also. here is a video in which we had a patient with a complex recurrent fistula in ano with multiple external opening and scared perineum. we did ligation of a fistulous tract in inter sphincteric space, reinforcement of ligated tract done using vicryl 2-0, as there was infected tissue in inter sphincteric space, tissue curated and left open to heal by secondary intention. all external openings widened and tracts were curated. LASER ablation of tract done. infant feeding tube left in the tract for postoperative irrigation. perianal infiltration of methylene blue with local anesthetic agent done for better postoperative pain relief. #fistulainano #complexfistula #LIFT #LASER #fistulasurgeon |
