The fun part about this case will be the immediate transformation from female-like breasts to a rockin’ natural male chest. The interesting thing about this case is the large amount of gland remaining after ultrasonic Vaser treatment. This gives Dr. Caridi the opportunity to showcase his tissue pullout techniques. The successful removal of the entire gland indicates that the patient will never have reoccurring gynecomastia. This is something Dr. Caridi is trying to teach - do not leave any glandular tissue behind. In this case, bilateral quilting sutures are used to limit dead space, the space left behind by the removed glandular tissue. Blood or other bodily fluids will naturally fill this space during the healing process. This accumulation of fluid can cause some post-operative complications, such as hematomas or seromas. Left around for long enough, this fluid can cause “hard healing” leading to excess scarring and, at times, contour irregularities. This is why it is important to use quilting sutures in cases where a large gland is present and removed. The risks include, but are not limited to bleeding, infection, hematoma formation, scar deformity, contour deformity, pain, paresthesias, numbness, asymmetries, dissatisfaction with results, seroma formation, recurrence of the condition and a host of other potential complications.
