| Video Discription | This case represents our 8th case that required re-laparoscopy for hemorrhage in 1330 consecutive sleeves. She was given 40mg of LMWH subcutaneously 2 h before surgery according to protocol. The patient did not have any bleeding tendency (normal INR, APTT, bleeding & clotting times), did not have hypertension but as can be seen, her staple-line was stitched over and over for quiet a number of times prior to the closure of this primary operation. At postoperative 10th hour, orthostatic hypotension and drop in Hb prompted us to perform re-laparoscopy. Tranexamic acid, protamine sulphate, vitamin K, 1 unit of FFP and 2 units of RBC was also given prior to re-laparoscopy. During the re-laparoscopy which will also be shared as Part 2, no obvious bleeder is found but a liter of blood is evacuated without any further sequela. |
