The New Kid has moved with his parents to South Park to escape his forgotten past. He quickly allies with Butters, Princess Kenny and their leader Cartman.
The main character, whom the player controls, is the New Kid—nicknamed "Douchebag" - a silent protagonist who has recently moved to the town. Befriending the local boys, he becomes involved in an epic fantasy live action role-playing game featuring wizards and warriors battling for control of the Stick of Truth, a twig that possesses limitless power.
The humans, led by Wizard King Cartman, make their home in the Kingdom of Kupa Keep, a makeshift camp built in Cartman's backyard; among their number are paladin Butters, thief Craig, Clyde, cleric Token, Tweek, and Kenny—a young boy who dresses as a princess. The humans' rivals are the drow elves, who live in the elven kingdom in the backyard of their leader, High Jew Elf Kyle; they also include the warrior Stan and Jimmy the bard.
The boys conduct their game throughout the town, the surrounding forest, and even into Canada (represented as a pixelated overhead 16-bit RPG). Locations from the show, including South Park Elementary, South Park Mall, the Bijou Cinema, City Wok restaurant, and Tweek Bros. Coffeehouse, are featured in the game.
The Stick of Truth features the following historical South Park characters: Stan's father Randy Marsh, school teacher Mr. Garrison, Jesus, school counsellor Mr. Mackey, former United States Vice-President Al Gore, the sadomasochism-loving Mr. Slave, sentient feces Mr. Hankey, City Wok restaurant owner Tuong Lu Kim, Stan's uncle Jimbo, Mayor McDaniels, Priest Maxi, Skeeter, Canadian celebrities Terrance and Phillip, the Underpants Gnomes, the Goth kids, the Ginger kids, the Crab People; the Christmas Critters, and local boys Timmy, Scott Malkinson, and Kevin Stoley.

The Fractured but Whole events occur one day after those of its precursor, The Stick of Truth. The main character is the same "New Kid", a silent protagonist. The children abandon their fantasy role-playing game to become superheroes and create a superhero media franchise. Unable to agree on who will get a film or Netflix series, the characters split into two rival factions, Coon and Friends and the Freedom Pals, and vie to create their own franchise first.
Led by the raccoon-clawed Coon (Eric Cartman), Coon and Friends operates from the Coon Lair, the Coon's basement. The group includes The Human Kite (Kyle Broflovski), the life-leeching Mosquito (Clyde Donovan),the super-fast Fastpass (Jimmy Valmer), the powerful Super Craig (Craig Tucker), and the New Kid. The Freedom Pals are led by the telekinetic Dr. Timothy (Timmy Burch), and includes Mysterion (Kenny McCormick), Toolshed (Stan Marsh), the weather-wielding Wonder Tweek (Tweek Tweak), and mechanized Tupperware (Token Black). Other heroes include the hacker Call Girl (Wendy Testaburger) and super-strong Captain Diabetes (Scott Malkinson). The heroes' journey brings them into conflict with groups including the local elderly population, ninjas, the sixth graders, the Hooters-parodying Raisins girls, hobos, Kyle's cousin Kyle Schwartz, and the supervillain Professor Chaos (Butters Stotch).
The New Kid can manipulate time with their farts; this ability can be used in battle to skip an opponent's turn, pause time to move around the grid and inflict out-of-turn damage, or summon a past version of the New Kid to join the battle. The New Kid's farting power can also be used to reverse time, fix objects, and pause time to avoid obstacles or fast-moving targets. Some areas of the town are inaccessible until the player gains specific allies who can use the New Kid's farting power to overcome the obstacle: Captain Diabetes can be sent into a rage that allows him to move heavy objects; the Human Kite can fly the New Kid to otherwise unreachable heights; Toolshed's sandblaster can clear paths that are blocked by lava that cause the player to combust and Professor Chaos can launch a hamster from the New Kid's rectum to reach and sabotage electrical panels.
