While for mild distal hypospadias a single stage urethroplasty has become the norm, very few hypospadias surgeons in India or worldwide do single stage hypospadias surgery for severe hypospadias such as scrotal Hypospadias. In the present case the child was 3 years old and also has a left undescended testis. As a first step left undescended testis was fixed in scrotum and then hypospadias repair procedure was started. Onlay island flap urethroplasty (Transverse Prepucial Onlay Island Flap Urethroplasty) was first described by Duckett in 1987 and gradually surgeons have adopted it for use in suitbale cases.
