| Video Discription | This Video is for healthcare professionals and gynecologists. Here , i am discussing the steps of a laparoscopic sacrohysteropexy using a macroporous polypropylene mesh - soft . The patient had undergone a modified shirodkars repair for nulliparous prolapse in another setup , 2 years ago . she developed genital prolapse over a period of 2 years with the cervix projecting beyond the introitus - 3-4 cms . After clinical evaluation and MRI imaging , isolated uterocervical prolapse was diagnosed and a laparoscopic sacrohysteropexy and excision of previously placed implant was planned . steps: 1. ensuring surgical feild exposure - sigmoid hitch 2. posterior inverted j shaped peritoneal incision in the right pararectal space and rectovaginal septum 3. excision of previous implant ant preservation of the hypogastric nerve. 4. anterior peritoneal incision- UV fold and bladder mobilisation 5..broad ligament windows lateral to the uterine vessels 6. Preparation of modified cruciate soft polyproppylene macroporus mesh 7.anterior anchorage of mesh to uterine cervix 8. posterior anchorage of mesh to cervical isthmus 9. extraperitonisation of the mesh 10. Anchorage of mesh to anterior longitudinal ligament over S1 - sacral promontorium 11. completion of the reperitonisation 12. hemostasis and release of sigmoid hitch. The prolapse was optimally corrected using the MIS approach. |
