Before and 6 months after photos included at the end of the video. A rhinoplasty, commonly referred to as a “nose job,” is a surgical procedure that reshapes the nose. It is one of the most commonly requested plastic surgery procedures and something that Dr. Caridi performs routinely at Westlake Plastic Surgery in Austin, Texas. Rhinoplasty is a true form of plastic surgery art. Only after 3 decades does Dr. Caridi consider himself a truly mastered surgeon for this critical undertaking. He explains that when you are considering a rhinoplasty, it is very important to seek out the most experienced surgeon as this surgery is measured in millimeters - changes that require masterful skill. This video includes a pre-operation anatomic analysis, surgical footage with voice overlay by Dr. Caridi, two post-operative exams - dorsal splint removal and six months post-operation, and before and after surgical / clinical photos. This patient presents with strong cartilage, bifid cleft columella, asymmetrical ala, dorsal hump, and thin skin that create very prominent nasal features in this patient. Thin skin is advantageous in that any small, finesse changes to the nose will be noticeable. He and the patient are targeting a nose that “fits her face better” by refining the tip with slight rotation, narrowing the base of the nose, and reducing the dorsal hump. Dr. Caridi will ensure normal breathing by examining internally and making any adjustments, if necessary, to the septum and/or turbinates. The procedure takes a few hours in the operating room under general anesthesia. Dr. Caridi uses the stair stepped incision on the columella to begin ""unroofing"" the nose - separating the cartilage and tip of the nose from the nasal bone under the nasal ridge / dorsal ridge / dorsum. The surgeon reduces the projection, or hump, of the nasal ridge using an incremental approach - gradual small changes overtime. The bone needs to be reduced with a rasp that chisels the bone. Dr. Caridi starts with a conservative approach and incrementally reduces the nasal ridge to a more refined appearance. He then performs a percutaneous osteotomy - breaking the nose bone to narrow the appearance of the base of the nose. The results are immediately visible with an ideal harmonious proportion between the proximal portion and the distal portion. The final step of the rhinoplasty involves closing the nose and analyzing the final appearance - the rotation of the tip, the relation between the tip of the nose and the dorsum, and dorsal alignment. Telfa surgical pads are inserted and skin glue is applied to hold the dorsal splint in place then a tip dressing is added to give the tip of the nose additional support. The splint is removed, swelling and breathing is checked, and the shaping of the nose is analyzed. The healing continues for several months with final results after two years. Six months after the procedure, the patient returns to the office again for her follow-up. Patient has full sensitivity in her nasal tip.
