| Video Discription | In this video, Dr. Solomon demonstrates the correction of ectropion in a patient’s left eye; the drooping of the patient’s eye can be plainly seen before surgery. This patient has undergone a blepharoplasty procedure in the past at another clinic and came to Dr. Solomon seeking to correct the results of that procedure. The intention of this revision blerpharoplasty it to lift the lower eyelid in order to prevent tearing and improve the aesthetics. Ectropion can occur after blepharoplasty surgery. Mild cases are called lower eyelid malposition where the white of your eye is exposed. In severe cases the lower eyelid is significantly lowered. The causes can be scar tissue resulting from prior surgery, excess surgical removal of skin or laxity of the lower eyelid. There are a number of methods to correct ectropion. In this case Dr Solomon tightens the lower eyelid in a patient who had lower eyelid surgery elsewhere and developed severe ectropion. Dr. Solomon begins the procedure by dissecting down to the orbital rim at the outside edge of the affected eye. He surgically dissects down to the bone on the side of the eye. He then creates a tarsal strip where he shortens the lower eyelid slightly. This will allow him to move the lower eyelid up, and improve its tone and position. He then places sutures along the side of the lower eyelid securing the tarsal strip and the orbital rim. By tightening these sutures, he draws the skin of the lower eyelid into position. The skin is then closed and sutured to allow for healing. Once the skin has closed we can see that the lower eyelid is in a much better position and is no longer lax.
