| Video Discription | Oral mucosa graft repair in redo hypospadias The inner lining of the cheek and lip is a different type of skin and is called oral mucosa. Oral mucosa does not grow hair, is used to remaining wet and heals very fast. After multiple hypospadias surgeries the native prepucial skin is unavailable for further repairs. In these cases, we can use oral mucosa graft. The graft is harvested from the cheek or lip depending on the length of the graft required. In redo hypospadias repair, cystoscopy is the first step done during the surgery. If the cystoscopy shows unhealthy urethra, then the unhealthy urethra is removed, and oral mucosa graft is placed in that area. The graft is sutured in the raw area and quilted on the bed. No sutures are placed in the oral cavity after harvesting the graft. Graft is placed only if the urethral bed has good blood supply. If the bed has fibrosis, then we close the skin and plan oral mucosa graft 6 months later. After oral mucosa graft repair there will be a dressing and catheter. Dressing and catheter are removed 7 days after surgery. For the initial 2 weeks an antibiotic ointment is applied over the graft site. After 2 weeks we examine the graft in the OPD and start steroid massage twice daily. Initially the graft is nodular but with daily massage it becomes soft like lip. Immediate complications will be infection of the graft. If the graft becomes infected, then there is a chance of graft loss. Late complications will be graft contracture. Sometimes the graft is hard inspite of steroid massage. In these cases, we will be unable to tubularize the graft and we may have to replace the graft partially or entirely. Stage 2 repair is done once the graft becomes soft. Graft tubularization can be done only once the graft is soft. It can take minimum six months and sometimes it may take 8-10 months for the graft to be soft. At hypospadias foundation, we treat children and adults with primary and redo hypospadias.
