| Video Discription | Pararectal Space Dissection for Endometriosis Amanda M Tower MD Fellow Yale University - Bridgeport Hospital Masoud Azodi, MD Yale University - Bridgeport Hospital Objective: To demonstrate a surgical technique for dissection of the obliterated posterior cul-de-sac for the minimally invasive surgeon. Methods: Video case presentation of a 38 year old gravida 1 para 1 who was referred for surgical treatment of endometriosis and dyschezia, treated laparoscopically with restoration of normal anatomy and ablation of endometriosis. She was evaluated in the office postoperatively with history and physical exam. Results: Endometriosis can lead to obliteration of the posterior cul-de-sac, causing pain, dyspareunia and dyschezia for the patient and presenting a challenge for the minimally invasive surgeon. The pararectal space is the potential space bordered medially by the rectum, superiorly by the sacral hollow, and inferiorly by the cardinal ligament, containing the uterine artery. This space can be utilized to aid in the dissection of the obliterated posterior cul-de-sac and restoration of normal anatomy. Endometriosis can then be resected from the back of the uterus once the rectum is mobilized. Conclusion: The pararectal space can be utilized to aid in the dissection of the obliterated posterior cul-de-sac and restoration of normal anatomy using minimally invasive techniques. |
