| Video Discription | 38 year old Lady presented with a right adnexal mass which was incidentally detected during routine USG, further evaluation revealed a CA 125 of 120 and 5 cm ovarian complex mass with surface papillary growth on MRI Pelvis. As staging work up was suggestive of Stage 1 disease and the size of the mass being 5 cm, can be easily retreived through vagina, she was planned for Robotic Right Salpingo Oophorectomy and Frozen Section Biopsy. Intra operative findings were similar to that of MRI. Posterior fold of Broad Ligament was nicked and retroperitoneal space opened, ureter identified and dissected out from the ovarian vessels. The ovarian pedicle dissected cranially as proximal to its origin as possible, cauterised with bipolar and cut. The mass then excised off its attachments from uterus at cornual end and ovarian ligament. A swab stick was then introduced into the posterior fornix to distend the vagina and posterior colpotomy done. An indigenous specimen bag introduced and the specimen was retrieved carefully without any manipulation and oncological spill. The frozen section report came as Low Grade Serous Carcinoma with a diffential of Borderline Serous Tumor, hence Hysterectomy with Right Salpingo Oophorectomy, Bilateral Pelvic and Para Aortic Lymphedenectomy, Omentectomy done via Robotic Platform using the same ports and single docking. Here, the procedure of Para Aortic Lymphedenectomy is demonstrated. Retroperitoneum further dissected from the root of mesentery cranially in a retrograde fashion. Inferior Vena Cava and Aorta identified. Lymph Node packet raised and dissected out from the underlying vessels using Split and Roll technique. Inferior Mesenteric Artery identified and secured. Lumbar vessels which arise from lateral aspect of the great vessels identified and secured with bipolar cautery. The entire lymph node packet put in a indigenous specimen bag and retreived from vagina without any spillage maintaining all oncological principles. |
