HISTORY: A 18 year old lady Severe saddle trauma, 2year ago with massive laceration and bleeding from the perineal area, in a local hospital hemostatic sutures were taken, followed later by severe scarring with cryptomenorrhea that needed repeated dilation with a uterine sound in the same hospital. She was referred to us for vulvo-vaginal reconstruction before here marriage. ON EXAMINATION: The labia majora were appearing normal The posterior vestibule was severely scarred with pin hole vaginal dimple.The vagina is obliterated by scar tissue, by passing a probe 2mm diameter, the dimple extruded pus. THE SURGICAL TECHNIQUE: We decide to perform dissection using our( NOVEL BALLOON DISSECTION TECHNIQUE); a Foley catheter 14-16 F will be insinuated into the vaginal dimple, and filled with 20ml saline. By making the perineum stretched by pulling the foley catheter the scar tissue will be dissected more easily and safely and reconstruction performed. THE KEY ADVANTAGES OF THIS TECHNIQUE ARE: 1- STRETCHING THE SCARRED AREA FROM INSIDE , COUNTER- STRETCH FACILITATE DISSECTION. 2-MAKES ADJACENT ORGANS AWAY ; AVOID INJURY OF RECTUM AND BLADDER/URETHRA. - Dissection was performed sharp and blunt according to the nature of adhesions -We had dissected adhesion in the midline medial to the perineal muscles - After freeing the posterior vaginal wall from the surrounding tissue, it had been advanced and sutured to the perineal skin without suturing the sub-cutaneous area to avoid affectin the future distention capacity of the neonagina. |
