| Video Discription | BACKGROUND: Endoscopic access in Gynaecological procedures provides an attractive avenue for surgical advancement, reducing the duration of the hospital stay, the recovery period and the discomfort to patients while increasing their satisfaction levels. However, potential injuries to the vessel, bowel and other organs during the blind entry process of the first trocar, coupled with the limited timely availability of experienced specialists in the non-metros for backup support intimidates many aspirants to adopt this safer surgical technique. OBJECTIVE: To showcase the utility of the Hasson method as an effective tool for the conduct of safe surgeries. METHODOLOGY: An audit of the over 500 Gynaecological surgeries performed over the past ten years at our hospital was undertaken for this study. RESULT: Herewith, I share my successful experience of the past ten years in adopting a charter for performing safe Laparoscopic surgeries in a suburban setting with limited availability of superspeciality skills. Using Hasson’s open method, in lieu of the first trocar, avoids the complications resulting from blind entry, even as we are able to enter the cavity in a few minutes. Over 500 gynaecological procedures (diagnostic laparoscopy, ruptured ectopic surgery, Laparoscopic Cystectomy, etc) – 333 of them being LAVH were handled independently – have been performed with no complication during the first trocar entry to report. CONCLUSION: Through usage of safe technique, with adequate training, methodical workflow and well-trained support staff, it is possible to do first-rate Laparoscopic work in a Tier 3 town with limited availability of experienced super specialists. Monitoring the various contemporary techniques adopted worldwide for the first entry, Hasson’s method is an eminent choice for practitioners in a Tier 3 town like ours to deliver quality outcomes. |
