| Video Discription | Dr. Daniel Kelly, Director of the Pacific Pituitary Disorders Center at the Pacific Neuroscience Institute, demonstrates the utility of the micro-Doppler probe during pituitary and skull base surgery to allow definitive identification of the paired carotid arteries. Endoscopic endonasal transsphenoidal surgery is rapidly becoming the preferred method for removing the vast majority of pituitary adenomas, craniopharyngiomas, Rathke's cleft cysts as well as some midline meningiomas and clival chordomas. In the removal of these tumors, the surgeons must clearly know the course of the carotid arteries which pass within millimeters of the pituitary gland as they course through the skull base to the brain. While the high-definition view provided by the endoscope and surgical navigation technology (like GPS) can help localize the carotid arteries, their precise course cannot always be certain with these techniques. As this video demonstrates, the micro-Doppler probe is an extremely useful device for real-time artery location to help avoid carotid artery injury and to facilitate maximal tumor exposure and removal.
