| Video Discription | Watch a complete nose job rhinoplasty procedure including the pre-op, the full surgery, the 2 week post op and the 8 week post op. What a transformation! Dorsal hump reduction is one of the most common reasons for patients wanting to get a rhinoplasty from our renowned rhinoplasty surgeon Dr. Robert Caridi at Westlake Plastic Surgery. On top of just wanting a smaller nose in general, our featured patient in this video is wanting a nose job to reduce the size of the hump on his nose. As Dr. C refers to it, this patient has what is called a high-tension nose – where the dorsum is dominant and overgrown, which creates a banana-like shaped hump that eclipses the tip. This is caused by a mixture of both bone and cartilage. Due to male noses generally being “stronger” (harder bones), the patient’s large dorsum hump and the artistic finesse required to make meaningful, but not overdone adjustments make this patient’s procedure a complicated case. The focus of the nose job procedure for this patient will be on first reducing the dorsum and then evaluating what refinements, if any, are needed on the nose tip, as well as a few internal adjustments. While a closed rhinoplasty (no incision/internal nasal exposure) is something that people are increasingly seeking, the truth is that this nose job technique is limited in what it can accomplish and why whether the open or closed technique is used is dictated by the extent of the surgery. During the nose job surgery, Dr. Caridi identifies that the two-paired tip cartilages of the columella are a little wide and his septum is slight overgrown at the end, but overall is not too bad. The procedure itself will follow the basic tenets of a rhinoplasty, firstly a reduction of the dorsum, septum and upper lateral cartilages, followed by septum bone reduction, tip refinement, necessary adjustments, fracture and final sutures. To ensure a nice, even and non “overdone” looking nose shape profile and due to the large amount of dorsum hump bone and cartilage needing to be removed, Dr. C makes cartilage and bone reductions in increments as opposed to in a “full bore” method. This appointment is commonly referred to as the "big reveal", as nurse Maria Noonan removes the patient’s splint and sutures, allowing the patient to finally see what his new nose and face looks like. Upon seeing his new nose and face for the first time, the patient is nothing but ecstatic that even with all the swelling his nose finally looks exactly how he has always wanted it. It certainly helped that the patient was vigilant about applying ice this entire time, as is evident from what can only be described as a prime example of due diligence during recovery, as there is minimal bruising. Later, at his 8-week post-op appointment, the patient is joined by Dr. C, who notes that he is looking exceptionally good even with the large amount of swelling that is normal for rhinoplasty patients. The swelling is located particularly in the tip and the area right above the tip and vigilant taping has helped keep the swelling down from what it could be. On top of ensuring that no issues have arisen since surgery that need attention, the 8-week post-op appointment is good time at which to administer steroid injections. Steroid injections are administered to help bunch up the skin, shrinking the areas of fullness that are left after a large reduction is performed, controlling where this skin settles and refining the results.
