We present a patient with acute appendicitis with a unique diagnosis of appendicitis. During the operation, a hydatid cyst of the cyst was detected in the echinococcosis of the appendix. Echinococcosis or hydatid cyst is a form of zoonosis caused by larvae from Echinococcus species, especially E. granulosus. The usual life cycle is between carnivore-defining hosts such as dogs or wolves and intermediate hosts such as sheep, goats and cattle. The adult worm lay eggs in the gastrointestinal tract of definitive hosts passing through the stool. People are incidental mansions that are infected by swallowing contaminated food.
