| Video Discription | Dr. Nicol Bush and Dr. Warren Snodgrass of the Hypospadias Specialty Center have moved into discussion of proximal hypospadias in their series, Hypospadias Masterclass. In session 7, they discuss the STAC repair - STraighten And Close. The STAC repair is the first - and most important - surgery in a 3 step repair for hypospadias with severe chordee. In this Masterclass, Drs. Bush and Snodgrass discuss why STAC is such a gamechanger for getting better results in the most difficult hypospadias cases. STAC sets the stage for better outcomes with the urinary channel by reliable making the penis straight, and getting symmetric penile skin surrounding the penis. Once the penis is straight with good skin, then making a urine channel with a staged graft repair becomes a relatively simple process. There is nothing simple about a repair when the penis is still bent and/or when short ventral penile skin is present. From a technical standpoint, this lecture covers their method of straightening the penis, covering the 3 corporotomies with dartos, tacking the penoscrotal junction at the base of the penis, and reconstructing the ventral penile skin and foreskin. |
