1. Are hypospadias common in adulthood?
a. Problem size: Hypospadias is the most common urological birth defect occurring in up to 1/150 newborn boys and this total to almost 80,000 new cases every year. There are a large number of adults living with hypospadias across the world with either unrepaired or failed repair. In India alone there are estimated 5 lakh adults who have either not received hypospadias surgery or have failed surgery in their earlier years and their families or they themselves have lost hope.
b. Hypospadias when not treated in childhood may have significant psychological consequences in adulthood. Many adults who were operated in childhood may not be satisfied with the results either due to interference in sexual function or with urinary problems.
c. Complications or residual problems: Recurrent chordee, Stricture, suboptimal cosmetic results, hair growth, diverticulum, urine splaying.
2. Can hypospadias be repaired in adults?
a. Yes, repair in adults is very much possible though it is more challenging as compared to in children. The techniques vary in adults and a large number may require staged procedure. Our main focus while treating hypospadias in adults is to give them a good cosmetic result with no interference in sexual function.
b. With failure rate being higher in adults, thorough planning starting from preoperative period, surgical techniques and then post-operative care becomes a very essential part to achieve best result with minimal complications.
3. Is the surgery for adult hypospadias same as for children?
a. Pre-surgery Counseling is very important as adults have to give consent for themselves and they have a lot of questions about recovery time and resumption of sexual activity.
b. Pre-operative tests are similar to those in children but in addition a chest xray, an ECG and fasting blood glucose tests may be required. Anesthesia in adults will be spinal or general based on the hypospadias repair technique.
c. During surgery, we take care of asepsis to prevent any infections. The post-operative recovery is slower in adults compared to children hence we do a wound inspection on post-operative day 4 and keep catheter for a longer duration.
4. What are the complications of hypospadias surgery in adults?
a. Complications rates after hypospadias repair are higher in adults when compared to children. The healing is slower in adults and the risk of infection is higher when compared to children. The erections in the post-operative period can be troublesome and we prescribe medications to prevent pain and interference with healing.
b. The duration of catheter stay is double in adults compared to children. Hence catheter and wound care has to be done for a longer time to prevent complications.
5. How much time does it take to heal fully & resume activities?
Hospital time- After the surgery patient is kept in hospital for 1-2 days for intravenous antibiotics which are changed to oral antibiotics and continued till catheter removal.
Urethral Catheter is generally left for a minimum of 14-21 days in distal hypospadias and 21-28 days for proximal and failed hypospadias.
Since most of the adults are in job or business it is expected they plan off from work for a minimum of 4 weeks. Work from home is usually not an issue which can be resumed immediately after surgery if the patient desires.
The adults who under hypospadias surgery have to refrain from sex and masturbation for almost 3-6 months after surgery. Once the surgery is complete resumption of sexual activity & marriage can be planned which will be around 3 -6 months later.
6. How are the outcomes for adult hypospadias surgery?
a. Every man has a right to seek treatment and get a functionally better penis. Outcomes for adult hypospadias are good in expert hands even after multiple failed surgeries.
