The techniques of UNIPORTAL spine endoscopy have been around since the late 1990s, and have been continuously developed through the decades to address the issues of spinal degenerative neurologic compression while also vastly improving on the minimal invasiveness of doing decompression. But the downside of uniportal spine endoscopy has always been the cost -- particularly, the cost of the equipement (i.e., the endoscopes of varied sizes themselves, as well as the compatible equipment used for handling vertebral bone & spinal soft tissue).
Enter the BIPORTAL technique of doing spine endoscopy. Based off a single type of rigid scope -- the 175mm x 4mm endoscope used in knee & shoulder arthroscopy -- this scope can be used just like the uniportal scope. However, this scope can be used along with basic spine instruments (the kind you use for oldschool open techniques) that are often already available in your OR suite. Hence, the considerable reduction in cost -- i.e., no need to buy new expensive equipment, since you already have everything you might need to do spine endoscopy.
Beginning around 2015, biportal spine endoscopy techniques were pioneered in South Korea as a means to expand the repertoire of skill sets and improve on the differing approaches to doing minimally invasive spine surgery. As with uniportal endoscopic work, biportal spine endoscopy provides the operator a most minimal bone window from which to effectively decompress the central spinal canal as well as the foraminal canals -- with the least amount of soft tissue interruption (muscle stripping & retraction), AND THE LEAST AMOUNT OF BONY & LIGAMENTOUS REMOVAL. This means that decompression can now be achieved in degenerative spine pathologies that, once upon a time, WOULD HAVE REQUIRED SIGNIFICANT REMOVAL OF CRITICAL STABILIZING STRUCTURES, and thus WOULD HAVE REQUIRED IMPLANT-BASED FUSION/FIXATION TECHNIQUES.
Thus, with endoscopic spine decompression techniques for degenerative neurologic compression, no implants are needed because no destabilizing bony/ligamentous removal need ever be done to achieve adequate decompression. No fusion means the spinal motion segment being operated on retains its range of motion -- and this is critical for avoiding accelerated degeneration of neighboring spinal levels (i.e., the ever dreaded "Adjacent Disc Syndrome", wherein the adjacent discs are forced to take up the extra movement loads that are no longer carried by the fused disc-facet level[s], thus leading to accelerated disc & facet joint degeneration of adjacent motion segments).
ADVANTAGES OF SPINAL ENDOSCOPIC SURGERY IN GENERAL:
-- preserved motion & preserved stability with adequate decompression;
-- no instability, no fusion required, no implants required;
-- Adjacent Disc Syndrome is avoided;
-- very minimal soft tissue interruption, faster healing, less post-op pain;
-- lower cost, shorter hospital stay, lower mobidity/complications...
ADVANTAGES OF BIPORTAL ENDOSCOPY IN PARTICULAR:
-- special equipment is cheaper to buy than with uniportal endoscopy;
-- bone & soft tissue equipment is readily available & also cheap;
-- wider range of independent movement of both scope & instrument;
-- equipment is generally shorter in length than in uniportal systems;
-- easier to familiarize with techniques, since it's just like arthroscopy;
