El Dr. Mikel Gorostidi, del Hospital Donostia de San Sebastián, nos introducirá a esta metodología que, a pesar de su simplicidad conceptual, tiene el potencial de transformar la forma en que se abordan y tratan ciertos tipos de cánceres. Se discutirán sus aplicaciones, beneficios y el proceso de identificación y extirpación de estos ganglios utilizando diversos trazadores. Esta técnica no solo permite una detección más precisa de la enfermedad, sino que también reduce la morbilidad asociada con procedimientos más invasivos.
VENTAJAS DE LA TÉCNICA
El ganglio centinela es el primer ganglio (o grupo de ganglios) que es probable que sea afectado por la diseminación del cáncer desde un tumor primario. El procedimiento busca identificar y analizar este ganglio en particular para determinar si hay células cancerosas presentes, lo que puede indicar si el cáncer se ha diseminado a otros ganglios o tejidos cercanos. Si el ganglio centinela no tiene células cancerosas, es menos probable que otros ganglios cercanos estén afectados.
El procedimiento se realiza inyectando un trazador (como el ICG) en el área alrededor del tumor. Este trazador viaja a través de los canales linfáticos y se acumula en el ganglio centinela, haciéndolo visible para los cirujanos.
El procedimiento tiene varias ventajas:
• Permite una detección más precisa de la enfermedad ganglionar.
• Evita la necesidad de extirpar múltiples ganglios, reduciendo la morbilidad asociada con la linfadenectomía completa.
La técnica del ganglio centinela ha sido validada y se recomienda para varios tipos de cáncer, incluyendo el cáncer de mama, vulva, endometrio y cuello de útero. Sin embargo, la técnica aún está siendo investigada para su uso en otros tipos de cáncer, como el cáncer de ovario.
En el procedimiento, es vital no dañar estructuras adyacentes y extraer el ganglio sin romperlo para obtener una evaluación precisa. Una vez extirpado, el ganglio se examina utilizando técnicas histológicas convencionales, y si es necesario, se utilizan técnicas adicionales de ultra estadiaje para detectar células cancerosas.
DISTINTOS TRAZADORES
Se han utilizado distintos trazadores, ya sean radiactivos o fluorescentes, y cada uno tiene sus propias ventajas y desventajas.
• Azul Tecnecio: es un radiotrazador
• ICG (Indocianina verde): Es un trazador fluorescente que, cuando es iluminado con luz de una longitud de onda específica, emite fluorescencia en una longitud de onda diferente, lo que permite su visualización. Una ventaja del ICG es su rápida eliminación del cuerpo y su excelente visualización intraoperatoria. Sin embargo, tiene la desventaja de migrar a diversas estaciones ganglionares, lo que puede complicar su interpretación en algunos casos.
• Verde de India: Es un colorante que se ha utilizado en algunos procedimientos médicos. Su uso no es tan común como el del ICG o el azul de Tecnecio.
• Trazadores híbridos: Como el campo de la medicina está en constante evolución, los investigadores están buscando formas de mejorar la eficacia de los trazadores combinando las propiedades de diferentes trazadores en uno solo. Estos trazadores híbridos pueden ofrecer ventajas en términos de visualización y precisión.
VÍAS DE DRENAJE
Es muy importante conocer que hay dos vías de drenaje principales, la vía parametrial alta, (Upper Parameterial Pathway), y la vía parametrial baja, (Lower Parametrial Pathway):
• Vía parametrial alta: cruza la arteria umbilical obliterada y se dirige hacia la zona obturadora. Luego, se extiende hacia la zona de los vasos ilíacos externos. Es esencial comprender esta vía, ya que puede ser la primera ruta de diseminación de ciertos tumores pélvicos.
• Vía parametrial baja: va por el meso uréter hacia toda la cadena hipogástrica, (o ilíaca interna). A partir de ahí, drena hacia la región pre-sacra y luego a los ganglios aórticos bajos.
VÍAS DE DRENAJE ALTERNATIVAS
Las vías de drenaje no son exclusivamente pélvicas. También hay una cadena de drenaje a través del infundíbulo pélvico. Esta vía, en principio, no es aceptada internacionalmente como necesaria, dado que el porcentaje de ganglios endometriales con afectación aórtica aislada es relativamente bajo.
CONCLUSIÓN
El ganglio centinela en cirugía laparoscópica ginecológica representa una técnica avanzada y precisa que permite la identificación y extracción del primer ganglio linfático al que se extiende un tumor.
