Despite surgery and several IVF attempts, she had not managed to have children. Medical treatment gave her only temporary and moderate relief of pain. On examination she had cystic formations resembling peritoneal pseudocysts, ovarian endometriomas and a left hydrosalpinx. She was also tender on vaginal examination with a right US ligament induration. Her AMH was low. She was planned for pelvic surgery knowing that her ovarian function would be further compromised. Oocyte donation was also discussed and accepted by the patient, as a possible fertility treatment, once her pelvic pain condition had improved. Conclusive points Re operating patients with severe endometriosis and previous laparotomies, represent extremely difficult and dangerous procedures The complication rates for bowel or ureteric injury supersedes the 5% mark in international publications Thus, one has to take full advantage of the laparoscopic superiority compared to laparotomy These are: 1) Higher magnification and optical resolution, especially with good quality screens and cameras 2) Correct use of diathermy instruments and their application! Be diligently hemostatic! 3) Open up bloodless spaces and restore your anatomy by dissecting organs free. Keep them out of harms way 4) Take your time. Examine and re examine your tissue before you cut! 5) make sure to assess bowel and ureteric integrity before you finish the case This patient made an uneventful recovery and left the hospital the following day.
