| Video Discription | Many mothers get engorged a few days after birth. Too often mothers are told that the engorgement is due to milk, but a lot of it is due to edema (water) in the breast tissue. Mothers should not get engorged to the point that a baby cannot latch on or cannot get milk from the breast. There are several reasons for this "overengorgement". 1. The mother receiving large amounts of intravenous fluids during the labour, birth and after the birth, often with oxytocin which causes retention of fluid. 2. The baby not feeding well in the first few days. When a baby latches on well and feeds well in the first few days, a mother may become "full" when the milk "comes in" but she should not get engorged to the point that the baby will not be able to latch on well and continue breastfeeding well. Reverse pressure softening, often with the mother leaning back to encourage drainage of fluid. The mother helps push the fluid out of the breast tissue as is shown in the video. The pressure should be maintained for at least a minute before moving the fingers around the areola so that all areas of the areola are drained. The baby should then be tried on the breast.
