| Video Discription | Cicatricial ectropion is a serious complication of eyelid surgery. Removing too much skin during the procedure is the main cause of this complication. Therefore, adequate repair involves adding skin to the eyelid. Recruitment of skin from the cheek requires very sophisticated and expensive surgery. Another option is to place a skin graft in this area. However, patients are often concerned about the cosmetic outcome of a skin graft. To get the best results, it is important to follow these surgical steps: First, start by ensuring adequate release of all the scar tissue. Appropriate sizing of the graft is important. Make a template of the defect prior to harvesting the skin graft. Skin should be harvested from the upper eyelids when possible; unfortunately, this is rarely feasible. The skin behind the ear is closest to eyelid skin in terms of its thickness and color. Make sure to harvest a thin skin graft and one that is slightly larger than the eyelid defect. Carefully excise the subcutaneous fat before placing the skin graft into the eyelid. Finally, remember to leave the eyelid on stretch to avoid early contraction of the graft and patch the eye for 72 hours. Although popular, a bolster is not necessary in these cases. The skin graft will become firm and thick at around week 4. Eyelid massages may help to soften the graft. The surgeon may want to inject anti-scar medications such as Triamcinolone and 5 FU.
