| Video Discription | Segmental Bowel Resection NOSE Technique For Residual Bowel Endometriosis 50 year old, post hysterectomised multigravida c/o lower back pain (10/10), loose motions and excess mucoid discharge associated with pain while passing stools (10/10) Patient had H/O 2 prior surgeries- Left oopherectomy with DIE excision and Laparaoscopic hysterectomy with excision of rectovaginal nodule and ureterolysis for endometriosis. MRI A long segment of anterior rectal wall thickening of length 5 cm with wall thickness varying from 1.2-1.5cm, lower end is 11-12 cm from anal verge and it invovles a circumference of around 40-50% of the bowel. Plan- Lap endometriotic exision with bowel resection anastomosis SOS laparotomy Intra-op- There is e/o- 1) dense adhesions between rectum ,the vaginal vault and lateral pelvic side walls 2)large 5x3 cm rectal nodule around 10 cm from anal verge. Complete adhesiolysis done, Bowel resection done to remove the rectal nodule and anastomosis done using staplers. Cut section of bowel showed more than 50% narrowing of the bowel lumen.
