| Video Discription | Prolonged and painful erection or priapism is a rare problem in urology, and it must be treated promptly. 30ye old male heroin addict showed at our Clinic with painful erection that lasted for 5 days. The erection occurred spontaneously in absence of appropriate sexual stimulation. We have attempted to achieve detumescence by injecting 2 ml adrenalin (1/100 000) in each cavernosal body. The injection was repeated 4 times at 20 min intervals. Blood pressure and heart rate were monitored during the injections. Since there was no effect, we have decided to perform shunt surgery. Shunt surgery for priapism diverts blood from the corpus cavernosum into another area such as the corpus spongiosum (glans or urethra) or the venous system (saphenous vein). T-shunt is a distal shunt and involves passing a number 10 blade vertically through the glans into the corporal bodies bilaterally, rotating the blade laterally 90° (to avoid urethral injury), and then removing it. This video shows step by step procedure of T-shunt surgery for priapism. The patient signed informed consent regarding publishing this video. |
