Lefort I, orthognathic surgery corrects the vertical direction, it means the height of the face. But also a correction of the sagittal sense, that is to say of the advancement of the jaws.
The primary goal of orthognathic surgery is to correct a pathological dental joint. In doing so, we re-harmonize the face by restoring it to adequate proportions. This surgery provides a gain on the ventilatory, masticatory and aesthetic function. It is the orthodontist who directs patients to the maxillofacial surgeon. The orthodontist prepares the teeth, he positions them so that the teeth fit together correctly during the procedure.
Under general anesthesia, osteotomies of the jaws are performed (bone segmentation) then the surgeon fixes the bone segments in the correct position using titanium mini-plates (biocompatible, light, resistant, without buzzing with metal detectors). They can be left for life or removed 1 year after the operation.
In practice all the surgery takes place in the mouth, leaving no scar on the face. Here I am making a sulcular route to expose the bone in the subperiosteum. The bone is cut (other patient on video) according to the so-called LEFORT I tracing, by ultrasound with a bone scalpel which is atraumatic and respects the soft tissues. Once the bone is severed, I perform the DOWN fracture which consists of lowering the maxillary bone. This part is delicate and technical, it requires at least training in maxillofacial surgery according to a course comprising 6 years of medical studies and 6 years of internship in surgery.
Once released, the dentate part of the jawbone can then be mobilized and placed in its new place. This new place having been computerized in advance with my protesist, who to make an occlusal splint adapted to the patient on which I will position the teeth intraoperatively before fixing my bone with the titanium plates.
In this patient, at the end of the operation, I perform a genioplasty, that is to say a modification of the chin to finish harmonizing the face and allow it to close its mouth correctly.
The patient has no pain with the appropriate analgesics, she presented with nausea the first 2 days, she remained in the hospital for 2 nights. Now the dental joint has been found, she will have to eat mixed for 1 month.
The aesthetic and masticatory impact is obvious.
La chirurgie orthognathique de type lefort I permet une correction du sens vertical, c’est-à-dire de la hauteur du visage. Mais également une correction du sens sagittal, c’est-à-dire de l’avancée des mâchoires.
Le but 1er de la chirurgie orthognathique est de corriger un articulé dentaire pathologique. Ce faisant, on réharmonise le visage en rétablissant des proportions adéquate. Cette chirurgie apporte un gain sur la fonction ventilatoire, masticatoire et esthétique. C’est l’orthodontiste qui adresse au chirurgien maxillo-facial les patients. . L’orthodontiste prépare les dents, il les positionne de façon que les dents s’emboitent correctement durant la chirurgie.
Sous anesthésie générale on réalise des ostéotomies des mâchoires (segmentation osseuse) puis le chirurgien fixe les segments osseux en bonne position grâce à des mini-plaques en visées en titane (biocompatible, léger, résistant, ne sonnant pas aux détecteur de métaux). Elles peuvent être laissé à vie ou alors retirées à 1 an post opératoire.
En pratique toute la chirurgie se passe dans la bouche, ne laissant pas de cicatrice sur le visage. Ici je réalise une voie sulculaire pour exposer l’os en sous périosté. L’os est découpé (autre patient sur vidéo) selon le tracé dit de LEFORT I, par ultrason avec un bone scalpel qui est atraumatique et respecte les tissus mous. Une fois l’os sectionné je réalise la DOWN fracture qui consiste à abaisser l’os maxillaire. Cette partie est délicate et technique, elle nécessite au minimum une formation en chirurgie maxillo-faciale selon un cursus comprenant 6 ans d’études de médecine et 6 ans d’internat en chirurgie.
Une fois libéré, la partie dentée de l’os maxillaire peux être alors mobilisé et placée à sa nouvelle place. Cette nouvelle place ayant été planifier informatiquement au préalable avec mon protésiste, qui à réaliser une gouttière occlusale adapté à la patiente sur laquelle je vais positionner les dents en per-opératoire avant de fixer mon os avec les plaques en titane.
Chez cette patient je réalise en fin d’intervention une génioplastie, c’est-à-dire une modification du menton pour finir d’harmoniser le visage et lui permettre de fermer la bouche correctement.
La patiente n’a pas de douleur avec les antalgiques adapté, elle présenté des nausées les 2 premiers jours, elle est restée 2 nuits à l’hôpital Maintenant l’articulé dentaire retrouvé il lui faudra manger mixé 1 mois.
L’impact esthétique et masticatoire est flagrant. Une prise en charge par kinésithérapeute spécialisé des mâchoires est réalisée des la sortie de l’hôpital. |
