| Video Discription | Patiente chez qui j’ai retiré il y a 4 mois un cancer des ¾ de la lèvre inférieure. Une reconstruction par un lambeau d’Eslander et un lambeau de Karanpendzic avec été réalisée (chirurgie détaillée dans la 1ere vidéo de ma chaine youtube que je vous invite à regarder). Ce jour, et grâce à une prise en charge par kinésithérapeute spécialisé en chirurgie maxillo facial, le résultat cicatriciel et fonctionnel est plutôt bon, il est limité cependant au niveau de l’ouverture buccale. Je vais donc réaliser sous anesthésie locale une commissuroplastie selon Gillies. La technique se trouve dans ce livre prêté par mon cher collègue le Dr ANDRIANAIVO Rabé. Technique en 4 étapes que nous allons suivre. Premièrement, il faut dessiner un triangle cutané dont le sommet correspond à la place de la nouvelle commissure. Une règle que j’ai apprise en dessin est que la commissure labiale, est alignée avec la pupille. On résèque ensuite la partie cutanée superficiellement comme ceci. Deuxièmement, la lèvre supérieure est reconstruite avec la partie juxtacommissurale de la lèvre inférieure. Il faut prendre un lambeau de lèvre rouge inférieure et surtout laisser le muscle de la lèvre inférieure en place. Ensuite on sectionne la partie endobuccale au milieu jusqu’à notre commissure. Troisièmement, la lèvre inférieure est reconstruite avec la muqueuse vestibulaire, on dissèque un lambeau endo buccal, qui viendra recouvrir le muscle laissé en place. On s’assure à chaque étape que les mouvements de tissus soit adapté à la patiente qu’ils soient au repos ou au mouvement. Quatrièmement, ce sont les finitions et les sutures, par exemple je réalise une plastie de la jonction entre néo-lèvre muqueuse et lévre restante pour diminuer la marche d’escalier pouvant être disgracieuse. Les sutures sont réalisées avec du fil non réorbable de type prolène 5-0. Voici un rappel de la lésion initiale de la patiente. Et voici la patiente en post opératoire immédiat avec une ouverture buccal bonne et un alignement des lèvres au sourire qui est satisfaisant. Ici le résultat à 1 mois post opératoire. Il est à noter le rôle essentiel de prise en charge par kinésithérapeute durant 6 mois et de façon intensive. Un renforcement musculaire du muscle orbiculaire des lèvres pourra alors permettre une compétence labiale optimale. Patient from whom I have removed a lower lip cancer 4 months ago. The technique can be found in this book lent from my dear colleague Dr ANDRIANAIVO Rabé. 4-step technique that we will follow. First, you have to draw a skin triangle, the top of which corresponds to the place of the new commissure. One rule I learned in drawing is that the lip commissure is aligned with the pupil. The skin part is then resected superficially like this. Second, the upper lip is reconstructed with the juxtacommissural part of the lower lip. It is necessary to take a lower red lip flap and especially to leave the muscle of the lower lip in place. Then we cut the intraoral part in the middle up to our commissure. Third, the lower lip is reconstructed with the vestibular mucosa, an endo-oral flap is dissected, which will cover the muscle left in place. At each step, we make sure that the tissue movements are suitable for the patient whether they are at rest or during movement. Fourth, it is the finishes and the sutures, for example I perform a plasty of the junction between the mucous neo-lip and the remaining lip to reduce the step of stairs which can be unsightly. Sutures are made with non-absorbable 5-0 prolene-type suture. Here is a reminder of the patient's initial injury. And here is the patient immediately after the operation with a good mouth opening and a satisfactory alignment of the lips with a smile. Here the result at 1 month postoperative. It should be noted the essential role of support by physiotherapist for 6 months and intensively. Muscle strengthening of the orbicularis muscle of the lips can then allow optimal labial competence. |
