| Video Discription | Eyelids surgery can be perform by 2 or 4. Upper eyelid surgery is done under local anesthesia and gives superb results with an almost immediate effect. Upper and lower eyelid surgery is a larger and more technical procedure usually performed under general anesthesia. The drawing of skin excess and fat pockets is adapted to each patient. After local anesthesia, incision crossing the orbicularis muscle, then the part of the anterior eyelid (musculo-cutaneous) is separated from the posterior eyelid (tarso-conjunctival). By this way you can go down in an NON vascular plane leading to the fat hernias. The orbital septum is then cut 3x over 5 mm to extract 3 fatty pockets (medial, central and lateral). La chirurgie des paupières du haut et du bas est une intervention plus importante et plus technique se faisant généralement sous anesthésie générale. Le marquage des excès cutanés et des poches graisseuses se fait de manière adaptée au patient. Le marquage se fait debout AVANT l’intervention. Le but étant des cacher chaque incision dans les rides du patient. Au bloc sous anesthésie générale, une asepsie est réalisé après protection oculaire par pommade vitamine A. La règle pour la paupière du haut de de conserver entre 8 et 10 mm par rapport au bord libre, le trait anatomique du patient est toujours retrouvé. L’incision est réalisée à la lame de bistouri puis l’excès cutané est retiré. Le muscle orbiculaire peut également être retiré chez des patients âgés avec ptose musculaire associée. La poche graisseuse interne est ensuite disséquée, coupée et retirée. Il est important de clipper avant de couper pour bien coaguler avant de relâcher cette portion graisseuse. Puis la suture est classique par un surjet intra dermique au prolène 5-0. Ici en vidéo chez un autre patient. Pour les paupières du bas, c’est une voie sous ciliaire, à 2-3 mm du bord libre. L’œil est protégé avec de la pommade vit A, puis les paupières sont suturées et la paupière du bas est tracter vers le haut. Après avoir infiltré à la xylocaïne adrénalinée, incision traversant le muscle orbiculaire, puis on sépare la partie de paupière antérieure (musculo-cutanée) de la paupière postérieure(tarso-conjonctivale). Cela permet de descendre dans un plan avasculaire jusqu’aux hernies. Le septum orbitaire est alors incisé 3x sur 5 mm pour extraire 3 poches graisseuses (médiale, centrale et latérale). Il est important d’avoir estimé le volume à retirer en pré opératoire pour ne pas retirer trop de graisse ce qui entrainerait des paupière creuses inesthétique. Elles sont alors clipper puis coupées puis coagulée. Il faut ensuite réaliser un lifting de la paupière sur la partie latérale. |
