| Video Discription | OPERATION STEPS 1. The peritoneum is opened along the right common iliac artery down to the external iliac artery. See and save the ureter. 2. The ovarian artery and vein crossing the common iliac artery will be ligated. 3. The round ligaments on each side are cut at the pelvic wall. Posterior and anterior leaves of the broad ligament are opened and entire external iliac artery is exposed. 4. The externel iliac vein is deviated laterally, exposing the obturatur fossa. All lymph nodes are removed. Infindubulopelvic ligamet is cut. Ovarian artery and vein are tied and cut at the pelvic brim 5. The ureter has been freed and the distal ureter has been tied and cut. The obturatuar fossa has been cleaned of all contents. 6. The internal iliac artery is ligated and cut. The distal portion of internal iliac artery is elevated and its branches are tied and cut. 7. Now at the anterior, Retzius space is opened by cutting adhesions between the bladder and pelvic wall. At this point any small vessels in the plexus of Santorini can be cut with ligasure. 8. At the lateral; in the paravesical space, the cardinal ligament is seen, cut with ligasure. 9. More deep at the lateral side, the ureterosacral ligament which includes hypogastric venous plexus, cut with ligasure. 10. Now internal iliac vein and artery cut and tied. 11. The specimen has been freed down to the levators and is retracted medially. 12. The uterosacral ligaments have been cut at the pelvic wall with ligasure. 13. Posterior dissection of rectum and Lateral wall dissection… 14. The remaining attachments are urethra, vagina and rectum 15. The urethra is cut near to its meatus and tied. The vagina cut and closed.
