| Video Discription | Basal ganglia lesions located in or adjacent to the lateral ventricle can be difficult to expose. Transcortical approaches or transsylvian approaches can be considered, but transgress a significant amount of normal brain and can disrupt important tracts. Transcallosal approaches transgress corpus callosum only. The contralateral transcallosal approach enhances the lateral exposure of a traditional transcallosal approach. With the patient's head turned 90º (lesion side up), the interhemispheric fissure opened with gravity retraction allows full visualization of the lateral ependymal surface of the contralateral ventricle. Furthermore, the approach to a left-sided lesion from the right spares the dominant hemisphere from retraction injury and potential sacrifice of bridging veins and is, therefore, safer than the traditional ipsilateral approach. In this 3-D video, we show the case of a 66 year-old ophthalmologist who presented with hemiparkinsonism and slow cognitive decline due to a 3 cm lesion located beneath the head of his left caudate nucleus. A contralateral transcallosal approach allowed the identification and complete removal of a cavernous malformation located immediately below the ependymal surface of his left lateral ventricle. The lesion extended all the way down to the carotid cistern and proximal Sylvian fissure, demonstrating how deeply one can reach through this approach. Six months after surgery, the patient recovered completely from his preoperative motor and cognitive deficits. |
