| Video Discription | Acute and chronic abdominal pain can be multifactorial. Whereas in most cases, an earlier diagnosis can be made with a few studies, some patients undergo multiple tests to identify its etiology. By providing a more rapid diagnosis and possibly simultaneous therapy, the surgical literature has repeatedly validated the role of laparoscopy in these patients. This modality is of particular use in cases where abnormal radiologic findings have been identified. This video is about a 33 year old woman who had presented with a left upper quadrant abdominal pain after heavy exercise 2 weeks earlier. Her history and exam were remarkable for a 10lb weight loss, focal upper abdominal tenderness, and a strong family history of gastric cancer. In the ensuing two months, she had had two hospital admissions, 3 CT scans showing a 2.8cm perigastric-perisplenic mass, 2 non-diagnostic CT guided biopsies; as well as a PET CT and an MRI showing mild metabolic activity and enhancement, respectively. Surgery had been consulted in the interim, recommending diagnostic laparoscopy which she refused at first. Once agreed to, she underwent a 55 minute operation where the mass was identified and easily removed. Her postoperative course was unremarkable with complete resolution of her symptoms. Final pathology was consistent with an infarcted hepatic choristoma. In cases of atypical abdominal pain, diagnostic laparoscopy can be both diagnostic and therapeutic and should be considered earlier in lieu of multiple additional expensive tests.
