| Video Discription | Hypospadias is a common congenital problem with an incidence of 1 in 150-200 boys. In hypospadias, the urinary opening is in an abnormal position which varies from distal penile, mid penile, proximal penile, penoscrotal, scrotal to perineal in position. Majority of these children have associated chordee. Chordee correction is the single and most important step during hypospadias repair. The methods for chordee correction can be broadly classified into dorsal and ventral procedure. Dorsal procedure involves tunica albuginea plication. Ventral procedure involves urethral plate division, proximal urethral mobilization, corporotomies and grafting the tunica albuginea. Type of procedure for correcting the chordee is decided based on the degree of chordee. Staged repair is better in cases with severe chordee to achieve better long-term outcome. The above video is of a child with scrotal hypospadias with severe chordee who underwent chordee correction by complete degloving, urethral plate division, proximal urethral mobilization, ventral corporotomies and dorsal tunical albuginea plication. Chordee was completely corrected with no residual chordee. Byar's flaps were created using dorsal prepuce completing the stage 1 repair. Staged repair has better long term outcomes in children with severe chordee. |
