| Video Discription | A 24 year old male patient born with a Tessier’s facial cleft deformity reported to Prof. S. M. Balaji, Craniofacial Surgeon, India to correct his deformity and residual defects. He had a macrostomia correction done elsewhere with large bilateral scars on the face. The patient had a profound scar in his cheek with the ramus of mandible severely affected and this was used as the entry point for correcting his residual deformity. Also, he wanted to correct his cleft nose simultaneously After standard preparation and administration of general anesthesia with nasal intubation, rib graft was harvested. The donor site was closed in layers after checking adequately. The recipient site was prepared for the surgery and accessed through the pre-existing scar. Prepared graft was shaped and positioned to correct the deformity. They were secured with screws. The nasal intubation was withdrawn and an oral intubation performed so as to facilitate correction of nose. A standard reduction rhinoplasty with medial and lateral osteotomy was performed through incisions made in the nasal mucosa. After due reduction, the incision was closed in layers.
