| Video Discription | The Shull repair is performed laparoscopically using the same technique for high-uterosacral ligaments (USLs) suspension as that described by Shull et al for the vaginal procedure. The laparoscopic Shull repair is performed following these surgical steps: step1, visualisation of the course of USLs by applying traction with forceps in its distal portion close to the vagina; step 2, dissection of vaginal vault; step 3, opening of the peritoneum of the broad ligament, with direct visualisation of the course of the ureter; step 4, exposure of the middle-upper third of the USL (at this level ureter is about 1,5cm lateral to the USL); step 5, placement of 6 suspending sutures for the vaginal suspension, each suture should transfix the higher portion of the USLs and should incorporate the peritoneum and vaginal fascia. |
