| Video Discription | The presented case represents Dr. Yerdel's 1303 rd consecutive laparoscopic anti-reflux procedure since 2004. There were no mortality & conversion. During the last 10 + years, some modifications in the technique surely occurred: 1- We abandoned doing Nissen's in favour of the so called Toupet's procedure . Because; we could not show any anti-reflux benefit in contrast to significantly increased rates of functional side effects & complications with Nissen's. ( J Laparoendosc Adv Surg Tech A 21;669-76.,2011) But we will surely never forget Dr. Nissen who did the 1st anti-reflux procedure in the world here in our city of İstanbul to a Turkish citizen. Prof. Nissen also trained some of our old-masters & we are all thankful for that. 2- 11 - 13 mm Hg pneumoperitoneum became a routine, recently. 3- Visiport became the 1st trocar insertion technique after an extensive bariatric experience, recently. 4- We do not try to do a 270 degree wrap, 180 will achieve the same.. 5- We do not use any fixation anterior to the mid-esophageal level & we have a reason to do so. Accordingly; we also abandoned putting a graft anterior to the oesophagus as well .
