| Video Discription | This young boy was born with bilateral cleft lip and palate. The primary surgeries were already done elsewhere. Now an Abbe flap surgery was planned to reconstruct his nose and upper lip defect. First an open rhinoplasty was done to lengthen the columella using a graft. The nasal tip was also modified. The margin of the flap was sutured to the base of the columella. The incision for the Abbe flap was marked. This technique was developed by American Plastic Surgeon Dr. Robert Abbe in 1898 for a complete philtral reconstruction. The flap is based on the labial branch of the facial artery. The forked W-shaped full-thickness flap was taken from the central lower lip, including the hair-follicles to ensure mustache growth. The flap was rotated across the mouth and the columella was reconstructed while the flap base remained attached to the lower lip. The surgical site was closed.
