| Video Discription | Adult chordee correction in severe chordee of redo hypospadias The single most important reason for failure in hypospadias repair is inadequate correction of chordee as the first step. At hypospadias foundation, almost 60% of adults or children who come to us with hypospadias complications also have a residual chordee. Hence, residual chordee correction is the first step in any redo hypospadias repair. Chordee correction in an adult is very different compared to children. Chordee correction is started by complete degloving, excision of fibrotic tissues and assessing the chordee. If the chordee is less than 30 degrees, then we plan a dorsal procedure. In children a simple dorsal plication at 12’o clock is sufficient to correct the chordee completely whereas in adult this is not enough. The dorsal plication cannot withstand the force which occurs during adult erection hence multiple plications are required in an adult to correct the chordee completely. These plications are done on the dorsal side on either side of midline at 4 places after raising the neurovascular bundles on either side. These neurovascular bundles have to be raised carefully because damage to these can cause erectile problems in the future. 16 dot plication is a dorsal procedure to correct the chordee in an adult. This may be in some cases be also accompanied by a ventral procedure which is urethral plate division and proximal urethral mobilization. While it may be possible to do a single stage oral mucosa inlay urethroplasty in some cases, in majority of adult redo hypospadias, a 2-3 stage procedure is needed for better long-term outcomes. The decision to do a single stage or a staged urethroplasty repair is taken in the operation theatre by the expert hypospadias surgeon after the first few steps of assessment.
